The score captures modern orchestral music typical of British dramas and is meant to reflect Sherlock Holmes' unstable mental state of mind. Drawing inspiration from Stravinsky's chaotic compositions, it incorporates simpler methods, like those in "Historie du Soldat Suite," with shifting time signatures creating a polymetric feel. Nathan’s score uses this in bar 35, shifting between 2/4, 4/4, and 1/4 time signatures spanning over various instruments, patterns, and rhythms, with cello and double bass providing the foundation, percussion, fleeting harmonies, and alternating piano and solo violin melodies. This mirrors Holmes' erratic violin playing and visual representation.
The piece uses a standard orchestral string arrangement, drum set, piano, and solo violin inspired by British drama soundtracks. The structure revolves around the main title sequence at bar 35, with other sections establishing, developing, and concluding the piece. Bars 7–26 feature chase music emphasising danger, with a rhythmical D major spiccato in double bass, higher spiccato in violins and viola, a repetitive cello melody, and erratic timpani reflecting Sherlock's chaos. At bar 23, the melody drops, allowing strings to create a soundscape before concluding the chase scene. Foley and sound effects then shift the tone, leading to tense strings and a choir buildup until the main title sequence.
The piece closes at bar 60, with sustained and rising tremolo strings to generate tension. Overall, it is a well-produced piece comparable to British TV drama soundtracks, capturing Sherlock's character brilliantly in the title sequence at bar 35.
